By choosing a coaching program you understand...

The agreement to participate in a coaching program with Christine DiDomenico includes the following:

Understanding the refund policy

• If you drop out before the first visit, 10% admin fee
• If you drop out after the first visit, 75% of the program is refundable
• If you drop out after the second visit, 50% of the program is refundable
• No refund will be given after the second visit has been completed.

Please note that this coaching program is not intended to treat or cure or prevent any disease and/or health condition. I cannot give advice on medications, and I always advise that you check with your primary care physician. Please see our Medical Terms and Conditions below for more information.

Understand that the services provided by Christine DiDomenico do not create a practitioner-patient relationship; it is purely educational and for informational purposes with Christine acting as a health coach.

Understand that Christine DiDomenico does not dispense medical advice nor prescribe treatment. Rather, she provides education to enhance my knowledge of health as it relates to foods, fasting, dietary supplements, and behaviors associated with health. While nutritional and botanical support can be an important complement to my medical care, I understand that health coaching is not a substitute for the diagnosis, treatment, or care of disease by a medical provider.

Understand that any testing provided by Christine DiDomenico are third party services and that Christine DiDomenico is simply interpreting the results of the testing.

Any participant acknowledges that the care received during these coaching sessions is separate from the care received from any medical facility and that any appointment is in no way intended to be constructed as medical advice or care. It is recommended and advised that the continued and regular medical supervision and care is done by a primary care physician.

Do not rely on this information as a substitute for, nor a replacement of, professional medical advice, diagnosis, or treatment. Any concerns or questions about health should always be in consult with a physician or other health-care professional. Do not disregard, avoid, or delay obtaining medical or health related advice from a health-care professional because of this health coaching arrangement. The use of any information provided by Christine DiDomenico is solely at the risk of the participant.

Neither the services provided by Christine DiDomenico nor any information provided is intended to suggest that professional medical care should not be sought, or that anyone should disregard professional medical advice. Nothing stated or made available through any services provided by Christine DiDomenico are intended to be, and must not be taken to be, the practice of medicine or the provision of health care treatment, instructions, diagnosis, prognosis, or advice.

Anyone under the care of a health care professional or currently use prescription medication, I should discuss any dietary changes or potential dietary supplements use with my doctor, and should not discontinue any prescription medications without first consulting my doctor.

Assumption of Risks and Releases:

A participant assumes the risks of the coaching program, including the risks of trying new foods, the risks of trying new styles of fasting and/or diet variation, the risk of trying new supplements, and the risks inherent in making overall lifestyle changes.

Christine DiDomenico is released from any and all liability, damages, causes of action, allegations, suits, sums of money, claims and demands whatsoever, in law or equity, which the participant ever had, now have, or will have in the future against Christine DiDomenico arising from the participants past or future participation in, or otherwise with respect to working with Christine DiDomenico.

Confidentiality:

Christine DiDomenico will keep the Client's information private, and will not share the Client's information with any third party unless compelled to by law or with the consent of the client. Client is subject to the terms of our Privacy Policy and Terms of Service posted on christinedidomenico.com.
